In case of emergency, do adult supervisors have permission to call emergency services and/or have your child taken to the hospital?

Yorktown Boosters, Inc. conducts events and activities where participants may be photographed. We may publish the names, photos and videos taken of students participating in a wide variety of activities and sports. Some of these images may be used for publicity purposes in booster, school, and APS publications, web sites, and social media outlets (e.g., Twitter, Facebook, Instagram, etc.) They may also be used in local newspapers and in TV and radio news outlets. Parents who wish for their children’s photographs to not be used for these purposes must contact Yorktown Boosters, Inc. via email or directly in person, so that we may ensure those photos are excluded from use.

Agreement to Release Assumption of Risk/Agreement to Hold Harmless *
I am aware there are certain inherent risks involved in participating in this Youth Soccer Clinic including, but not limited to the risk of personal injury from participation in this program along with the risk of theft or of damage to my property.
Additionally, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed or infected by COVID-19 by participating and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 while participating in athletics may result from the actions, omissions, or negligence of myself and others.
I am the parent or legal guardian of my respective clinic participant, a participant in the Yorktown Youth Soccer Clinic. On behalf of myself and the participant, and in consideration of my child/guardian being granted permission to participate in these activites and to use the facilities of the Arlington Public Schools, I, on behalf of myself, my executor, administrators, heirs, next of kin, and successors, hereby covenant to hold harmless and indemnify the Arlington Public Schools and all its officers, departments, agencies, agents and employees from any and all claims, lessees, damages, injuries, fines, penalties and costs (including court costs and attorney's fees), charges, liabilities, or exposures, however caused, resulting from or arising out of or in any way connected to my or my family's participation in the clinic. I have read and understand this HOLD HARMLESS AGREEMENT and agree to its terms.
